ドライソケットとは
ドライソケットとは線溶性骨炎または歯槽骨炎とも呼ばれます。
抜歯をした穴は通常、血栓や歯肉で覆われずて治っていきます。
しかし、ドライソケットではさまざまな要因で正常な治癒が得られず
抜歯した穴の骨が露出した状態のままになってしまいます。
抜歯の数日後から痛みなどの自覚症状が出始めます。

抜歯してから数日後から痛みが強くなることがあります。
頭痛を伴うケースもあります。
抜歯した直後よりも数日後のほうが痛くなってきた場合ドライソケットかもしれません。
また、抜歯した部位に骨の一部がむき出しになって見えてくることがあります。
ドライソケットの原因
ドライソケットの原因は諸説あります。
かつては抜歯部位に細菌感染することが原因とも言われていました。
しかしさまざまな研究から現在はその可能性や影響は少ないと言われています。
現在のエビデンス(根拠に基づいた)ではドライソケットの原因は
・局所的な虚血(喫煙・経口避妊薬)による不完全な血栓(かさぶた)
・線溶系の亢進による血栓の融解(血を溶かす作用が強まりかさぶたが溶ける)
・強い力で抜歯することで周囲の骨芽細胞の壊死する(骨が再生するサイクルへのダメージ)
つまり、血栓(かさぶた)がうまく出来なかったり、骨の再生サイクルのダメージが大きいと発生するということです。
ドライソケットの治療法
・グルコン酸クロルヘキシジンを含む殺菌力の高い洗口剤
・生理食塩水による局所洗浄
・再掻爬(そうは)
抗生剤の使用はほとんど効果は認めないというデータもあります。
抜歯した歯科医院に早めに相談しましょう。
ドライソケットについてのまとめ
喫煙や経口避妊薬の服用がある場合、必ず事前に歯科医師に確認をすることをおすすめします。
